MEMBERSHIP DETAILS
Either call us directly or visit one of our clinics to fill out the required form. We can also email this to you by request.
There is a once off establishment fee of $7.70 plus a 3.3% fee on each transaction.
Yes, if it is required we can adjust your treatment plan and change you over to a new plan to suit your skin needs.
We may allow you to pause your membership for 1 month throughout the duration of your membership, this is at the discretion of management and must be pre organised in advance.
All memberships are a lock in contract. If you wish to cancel your membership early the remaining balance will need to be paid out in full. For exceptional circumstances please contact the clinic directly and our management team will assess your application to cancel.
First payment is deducted the day after signing up for your Skin Forum membership unless specified otherwise.
All memberships including the $7.70 joining fee are non-transferable and non-refundable.
Standard booking policy and no-show policy applies to all appointments.
We cannot guarantee an availability for a particular day, time or therapist. For this reason, we encourage clients to book in advance.
Discounts are only available on full priced products. If already discounted, the lesser of the two prices will apply.
Monthly treatments must be used every calendar month.
Monthly membership fees cannot be carried over from month to month.
Skin Forum reserves the right to change or terminate the Membership program at any time with a minimum of one month’s notice provided to all members.
Skin Forum reserves the right to refuse or cancel a membership when a member is not abiding by the terms and conditions.
